How about River City Ransom for the NES? You run around and beat people up, taking their money to buy food.... Ok, sounds odd and it is, but it's a cool action-fighter type game along the lines of Final Fight or something, but with AWESOME (i.e. horrible) NES graphics. I believe there's some type of plot involved too.

And then there's the semi-spinoff Super Dodgeball or something like that, with the same sprites. You go from country to country knocking the crap out of people with a dodgeball. It's beatable in like 15 minutes.

Pilotwings for the SNES, cool game, you have to get your licence on a series of stages by flying planes, skydiving, hangliding and jet-packing. At the end there's a cool helicopter stage where you have to inflitrate an enemy base and blow up their AA batteries. Plus fun minigames along the way.

And who can forget Excitebike for the NES? I loved that game..

Some more, if for no other reason than to solidify the notion that laszlow is a bum who leaves his forae on all day and who posts too much.

Weird but not really underrated: Katamari Damacy. You roll a ball around to pick up stuff. You have to play it to understand.

Underrated but not as weird: Megaman Legends 1 and 2. Basically a Zelda-style 3D adventure game, WAY deviating from traditional Megaman but in an acceptable way (unlike X7).

Both underrated and weird: Powerstone 1 and 2, but mostly 2. Party fighter that's both better and crazier than Super Smash Brothers. Yes, you heard me correctly.

Oh, and I thought Sudeki sucked. It was a button masher that had decent visuals but little else. It was surpassed by Fable because Fable was a solid action-RPG that was overhyped didn't have all that much to surpass.

Edit
River City Ransom rules. There was a decent GBA port that came out last year. Pilotwings was fun, but I preferred the N64 version. None of those games are underrated, dude, they all seriously rock.

Super Dodgeball was the coolest game ever. The best stage was Iceland, where the court is made of ice and penguins are watching the game.

It was supereasy, but if I remember correctly there was a two player game that made it all worthwhile.

Quite possibly Dynamite Headdy is the wierdest underrated game I've ever played. It revolves around this character named Headdy (yes it's spelled right) as he goes on a quest to stop a man named Dark Demon (a very powerful overlord with a scary Smiley Face on his chest). "Gee BM, what's so wierd about that?" Glad you asked that, Heady's Head is detachable and he uses that as his weapon, he also gets powerups for his head that can turn it into a: Hammer, a Mace, Split into three, shoot stars, shrink him and many more. And the whole game takes place as a stage-like play which is divided into acts and intermissions. AALLLSSOOO, this evil kitty runs into you numerous times in different mechs to try to stop you, let's not forget the battle between a Giant Wind Up Dog, a cross dressing manaquen, a puppateer who's lower body turns into a puppet and a giant baby face which ages as you fight it. Then there's the silly knock off names (some of which are movies) for some of the levels like: Toys in the Hood, Fatal Contraption, Fly Hard, Illegal Weapon, Stair Wars, Terminate Her Too, Headdy in Wonderland, Twin Freaks, well you get the point. Dynamite Headdy was released for the Sega Genesis and didn't sell alot (like Gunstar Heroes, it's even made by the same company), but believe me this game is fun, creative, witty and addicting.

Also:
Mario Paint
Streets of Rage
Little Nemo the Dream Master
The Magical Quest of Mickey Mouse
Super Ghouls and Ghosts
Mighty Final Fight
Mighty Morphin Power Rangers
Lemmings
Wild Guns (not Arms, Guns)
Dr. Robotnik's Mean Bean Machine (A hidden gem)
Rocket Knight (The faithful tale of a possum in a rocket-powered suit of armor)
Revenge of Shinobi
Animaniacs
Robocop Vs. Terminator

I got tons of 'em
